Practice information is carried on the front and back covers of "Your
Doctor". Most practices use the front cover for their practice name,
address and contact details, together with information on the doctors
and staff as well as surgery hours and services. The back cover is
generally used for information on fees, other practice policy matters
such as as repeat scripts, sickness certificates and general news on
staff movements and celebrations.
However, the information that goes here is entirely up to you. Below is
an example of a front and back cover providing essential contact and
staff information, some important surgery policy matters, an
after-hours contact notice and surgery hours.

Practice logo if required or use our standard chart and stethoscope graphic. Use the AGPAL or GPA logo if Accredited.

access to doctors by phone, feedback avenues and complaints resolution.

We suggest you use the following checklist to safeguard against any oversight.
You don't have to provide information for every item, or you might want to provide something that is not on the checklist.

- The name of the practice.
- Full address and contact numbers (phone, fax, email, web-site).
- Names, qualifications and areas of special interest of each doctor.
- Surgery Hours and Services, including Appointment requirements, After-Hours and Home visits arrangements.
- Fee schedule and/or Fee policy.
- Social items such as staff movements, holidays, births and marriages, etc.
- Contact name for the practice, if different from the person sending in the Practice Information.
- Please be sure to confirm how many copies you require, and how often you require them.
